Below is the total monthly cost for coverage for eligible retirees & spouses. The below rates are calculated per person, per month and listed based on age. NOTE: Enrollees do not need to send payment with enrollment forms. Coverage coming soon to residents in FL. Coverage is not currently available to residents in WA.

All retirees, spouses and surviving spouses age 65+ and enrolled in Medicare A & B

How do I enroll?

Complete The Hartford coverage enrollment form via paper document or online. Be sure to fill out the entire form and return to Benistar via online, mail or email.
Important, be sure to:

  1. Include all your personal information including SSN and Medicare ID number
  2. Put in a coverage effective date
  3. Check the medical plan you want to enroll into
  4. Sign and date the form

Does The Hartford retiree medical coverage have networks?

NO, you can go to any doctor in the U.S. who accepts original Medicare.

What information do I provide at my doctor’s office?

You should provide your Medicare ID and your Hartford ID cards.

What if my doctor does not take Hartford coverage

Please tell your doctor that Hartford’s coverage works like a Medicare supplement.  Medicare is primary and Hartford is secondary…Hartford’s coverage works everywhere that Medicare is accepted.

Does The Hartford’s coverage include prescription drugs (Rx)?

It does not…this is a medical plan only.  For Rx coverage you will need to enroll into a separate individual Medicare Part D plan.

How are my medical claims filed?

As Hartford’s coverage is integrated with Medicare, all claims should be filed directly from your provider to Medicare.  Hartford will pick up the claims electronically from Medicare. Once Medicare and Hartford make payments, you will receive an Explanation of Benefits (EOB) with all the details and a request for copayment if a payment is due.

How do the Non-Medicare benefits work? Do I need a referral?

For the non-Medicare covered benefits (Chiropractic and Acupuncture), you can receive treatment from any licensed provider. You do not need a referral…you can go when and where you want.


Chiropractic and Acupuncture claims – although these benefits are not covered by Medicare, the provider can submit the claim to Medicare and it will pass through to The Hartford for payment.  If your provider will not submit the claim to Medicare please call customer service to file a claim.

Benistar is Hartford’s plan administrator handling Customer Service, Enrollment, Billing and I.D. cards.  You can contact Benistar customer service at 1-800-236-4782.
